Welcome to Summer:2018. We’re very happy to have you as part of our incredible staff. There’s a bit of paperwork that must be completed before camp begins. The next few pages will take you through information for form W-4 from the IRS, form MW-507 from the Maryland Comptroller, form I-9 from The Department of Homeland Security, and a medical information form. All information is secretly captured and immediately transmitted to camp on submission. Please download the PDFs of the W-4 and MW-507 worksheets, above, before you begin. We will only ask for information common to all of the forms one time and then automatically populate similar fields across the forms on submission.
As part of form I-9, camp must use certain items to verify employment eligibility. You may choose from any of the listed items. Please be aware that should you use an item from “column a”, that expires, such as a passport, you must complete a new I-9 before working at camp after that expiration date. Items in “column b” that expire, such as a driver’s license, remain a valid form of identification and will not require submitting a new I-9 on expiration.
